Community reintegration planning that includes a wide spectrum of individualized counselling provided by professional CRF Case Managers. Assistance with resume writing, job search skills, and preparing for employment interviews. Visiting Physician, Community Health Worker, Nurse Practitioner Health Worker and Hepatitis C Team.
